On April 15, 2013, a temporary worker was fatally injured by getting caught in a mixing machine at Henkel’s adhesive manufacturing plant in Bay Point, CA. This incident was investigated by California’s OSHA, (Cal/OSHA). Summarize this case and show how the accident could have been prevented.
